NOTE: THE SCHOOL/ORGANIZATION IS THE REGISTRANT NOT THE INDIVIDUAL PLACING THE ORDER

• First Name and Last Name: Name of School/Organization
  • Example: Walnut Grove Secondary School
    • First Name: Walnut Grove; Last Name: Secondary School, Secondary or SS
• Birthdate: 1940-01-01 (default date)
• Email Address:  Email of Contact for School/Organization 
• Phone Number: Phone number of Contact for School/Organization
• Remaining Fields: complete as required

EMAIL CONFIRMATION - Upon successful completion of the scorebook order (and payment), an email confirmation will be sent to the email address provided in the scorebook order registration.  This is not the notification for the order pickup.  Note: the email may be sent to the junk folder depending on the email filter.  Please contact Basketball BC if you do not receive the email confirmation.
